PATH 1. COMMUNITY HEALTH IMPROVEMENT (1 POINT)

Demonstrate 10% city-wide improvement in one health status indicator from a user-defined

baseline year. The selected baseline year must fall within the last five years.

OR

PATH 2. REDUCTION IN DISPARITIES (2 POINTS)

Meet the following requirements:

a. Comply with Path 1

b. Demonstrate that any two priority neighborhoods and/or populations are within 5% of the

city-wide average for the same health status indicator.

c. Verify that key population characteristics have remained within five percentage points of

preintervention levels within the last five years.

AND/OR

Option 3. Food and Nutrition (1 point)

Adopt and implement city-wide policy or programs that support food access and community nutrition by

addressing one of the following strategies:

a. Affordable and healthy food access and nutrition programs in low-income neighborhoods

b. Free or reduced school lunch offered throughout the year to all students

c. Procurement policy for healthy and fresh food, which includes all food purchased and served

at public institutions, such as schools and hospitals, or public events

d. Policy or program that promotes the creation of fresh food retail initiatives
